In-Home Hospice Care Across Southern California

Hospice is for someone whose physician and our hospice medical director certify that, if the illness follows its usual course, life expectancy is six months or less. You do not need a referral to call us. We can talk with your loved one’s doctor and help confirm eligibility.

Covered care for eligible patients includes nursing visits, hospice aides, social work, chaplain support, and medications, equipment, and supplies related to the hospice diagnosis. It also includes respite care for caregivers and bereavement support for your family. Medicare may ask for a small copay on some medications and on inpatient respite, and we explain any cost before care begins.

Visits follow a plan of care built around your loved one’s needs, and they increase when needs change. Between visits, a nurse is on call 24 hours a day, 7 days a week, so you can reach someone whenever a question comes up.

Yes. Your loved one can leave hospice at any time, for example to pursue treatment again or if their health improves. If they qualify later, they can return to hospice care.

Your team includes a hospice physician, registered nurses, hospice aides, a social worker, a chaplain, and trained volunteers. Your own doctor can stay involved and work with our medical director on the plan of care.
